Clinical Care Coordinator, RN

About the Role

We are looking for a committed Clinical Care Coordinator to assist medical providers with patient intake processes and workflow management. This is a full-time, remote position focused on optimizing medical practice operations by easing administrative workloads, allowing providers to prioritize patient treatment. You will act as the main liaison for clinical associates, ensuring that all patient records are precise, thorough, and meet healthcare regulatory standards. In this collaborative professional setting, you will manage patient triaging, verify insurance details, and oversee care transitions for those utilizing home health services. This role emphasizes clinical proficiency, high-quality care, and effective communication, where you will improve patient results and lower readmission frequency through proactive management of clinical handoffs and risk assessment.

Requirements

• You are required to have a Registered Nurse status or a solid clinical foundation backed by a nursing or medical degree, with an active nursing license being preferred.
• You need substantial background in home health or home care settings, demonstrating the ability to use sound clinical judgment when reviewing referrals and triaging patients.
• You must possess high attention to detail and the ability to independently evaluate patient symptoms and spot potential medication errors or complications.
• You must be fluent in English, both orally and in written form.
• You need a reliable computer setup featuring a webcam, a noise-canceling headset, and a high-speed internet connection.
• Experience with the Kinnser EHR platform is an advantage, and familiarity with working in a remote capacity is highly desirable.
• You must pass a local police clearance or NBI check before beginning your engagement.

Responsibilities

• You will manage, record, and triage new referrals originating from community sources, physicians, and hospitals, confirming that all clinical paperwork is correct and thorough.
• You will check insurance statuses, evaluate patient eligibility, and confirm the availability of services within the designated region.
• You will mitigate patient risk by detecting clinical issues—such as respiratory needs, IV therapy, or complex wounds—and notifying leadership of high-risk scenarios.
• You will ensure seamless care transitions by delivering thorough clinical handoffs to field care teams and nurses, while also overseeing documentation regarding care holdups and discharge preparations.
• You will provide ongoing patient monitoring via weekly check-in calls to spot any symptom shifts or issues with caregivers, logging all details and confirming that patient acuity remains suited to nursing skills.
